i saw this problem and i ignored it at 1.03 but still on 1.04 and im suffering of it
it happen with cube shape, if i made it very slim but plane is ok
i tried Stefan glass materials and i made my own and still.. look picture will explain

i happens when i make a cube and make the both sides touch each others to be like plane, but its ok i think when you make it high a bit it will work fine

looks like some bad overlapping polys.
you could repait geometry or try to set the secondary ray bias(under option tab) to 0.1 instaed of 0

I have seen your file: try to use a cube with Y=1 cm or a plane!

The two faces of the cube one intersect the other! ;)

didn“t test it, but I am not surprised. This is a bad thing in almost every render software I know.

When you need it flat, why not use a plane object instead?

i opend the scene, and yes its true what mr. brown and the other guys said: you have to give the cube some thickness in y direction!
then everything is fine!

The reason is.....

If you use the volume in refraction layer you cannot use the object with "volume".

When you have an object with 2 faces the volume is real.

When you have an object with 1 face (plane ect...) for simulate the volume you need chek the "volume" in refraction layer.

volume should be ever ON in glass.
